Why is that it is so difficult to fix your name if they have it misspelled or you have no name in your Birth Certificate? It is not your fault anyway.
I am planning to get a passport so I could travel abroad and I was asked to present an NSO Certified copy of my Birth Certificate. So, I went to National Statistics Office(NSO) in Quezon city to get one. I was surprised that it appears that I have no name on my record. Then I was asked to go back to the Municipal Office of the City where I was born to get a Suplimental Report so I could fix it.
I went to Manila City Hall Records Section to verify. Then yes, I really don’t have a name registered on the record. So I go to the next step to get a suplimental report. Now, I’ve got a new problem. They are asking me to present some legal documents with my parents signature as a proof that I can use my father’s lastname wherein that’s what I’ve been using since then.
And there’s another problem. I have used my father’s lastname in all of my school records, legal and government documents (SSS, Pag-Ibig, Tax Records, Driver’s License, PhilHealth, My housing loan and title, bank records and employment records). Sad thing is, I don’t have any documents with my father’s signature that will serve as a proof so I could continue using my last name.
I talked to a lawyer and I was advised to just use my middle name (My mother’s maiden name) as my last name to make it easier. And yes, I can probably do that, however, I am concerned about my legal records. And now I’m doomed!
I am still trying to find a way to find a solution for this problem. And I’m hoping that I can find one.
And for those people who has the same issue or a birth certificate problem, I suggest that you fix it immediately. Don’t wait before it’s too late like what happened to me.
